If you are having a crisis and you have a mental health provider, please call them first – they all have after-hours contact information.
Otherwise, call the following numbers:
- Psychiatric Crisis Center (PCC): 503-585-4949
- Marion County Youth and Family Crisis Services 503-576-4673
- 503-623-2989/office hours
- 503-581-5535/after hours through NWHS Crisis Hotline
The Emergency Room should be used only if you can’t reach your Provider or Crisis Services and you feel like you might harm yourself or someone else.
Developing a Crisis Prevention Plan
Crisis Prevention Plans are meant to help individuals, their support persons and providers prepare for times when life seems too hard to manage. This plan is designed to support conversations about what would help when additional support or action is needed. We recommend you talk about this plan with a person who knows you well enough to understand your strengths and your challenges. It is best to complete this plan when you are feeling better about yourself and your life.
Use your own ideas and make this plan truly fit you. You can fill out this plan by yourself, with a peer support person, a family member, a friend or with a mental health provider.
Please go to the Forms Section of the website to download an Adult Crisis Plan or Child & Youth Crisis Plan.
